Mentis Assist’s Access and Support Program provides personalised, one-on-one assistance to help individuals of all ages connect with the services and supports they need. Whether you're navigating the aged care system, exploring NDIS eligibility, or looking to connect with your community, our Access and Support worker is here to guide and advocate for you every step of the way.

What We Offer

Our Access and Support Program provides:

  • Individualised support to identify your specific needs and goals
  • Help navigating complex service systems, including aged care and disability support
  • Collaboration with your existing supports, such as health professionals and family members
  • Advocacy to ensure your voice is heard and your needs are met

How We Can Help

An Access and Support worker can assist you with:

  • Contacting and advocating for you with My Aged Care, including support with referrals under the Support at Home program
  • Accessing local council services and a range of allied health providers
  • Understanding and exploring NDIS eligibility and referral pathways
  • Connecting to community-based programs such as Social Support Groups
  • Providing advocacy to ensure you receive appropriate and respectful support

Eligibility

The program is open to:

  • Adults aged 65 and over who are socially isolated, financially vulnerable, or experiencing mental health challenges.
    • Participants in this age group must be referred through My Aged Care under the Support at Home program.
  • Adults under 65 who are eligible for Home and Community Care (HACC) services
