The Linkages program offers support to people aged under 65 or 50 for Aboriginal or Torres Strait Islander living with complex mental and physical health needs.
A care coordinator will work with you to assess your needs and goals before putting a care plan together that incorporates support services and community connections.
- Referrals to various allied health services including OT, physiotherapy, psychology;
- Referrals to local council for HACC assessments and services;
- Assistance with accessing NDIS or My Aged Care supports;
- Nursing;
- Domestic assistance;
- Maintaining/building life skills;
- Community connections.
Eligibilitiy
You may be eligible for Linkages if you require assistance due to physical and or psychosocial functional impairment related to disability, chronic illness or short-term health needs, and could benefit from assistance with daily activities.
