Mentis Assist’s Linkages Program provides tailored support for adults under 65 (or under 50 for Aboriginal or Torres Strait Islander peoples) who are living with complex mental and physical health needs. The program offers short term support to improve independence and quality of life by coordinating a range of health and community services to meet each individual's specific goals.
A dedicated Care Coordinator will work closely with you to understand your needs, develop a personalised care plan, and connect you with appropriate supports to help you live safely and confidently in the community.
What We Offer
The Linkages Program can provide access to:
- Referrals to allied health services such as occupational therapy, physiotherapy, and psychology
- Referrals to local councils for Home and Community Care (HACC) assessments and services
- Support to access NDIS services where appropriate
- Nursing support
- Domestic assistance to help with household tasks
- Support to maintain and build life skills for greater independence
- Community connection opportunities to reduce isolation and enhance wellbeing

Eligibility
You may be eligible for the Linkages Program if you:
- Are an adult under 65 years of age (or under 50 if you are Aboriginal or Torres Strait Islander)
- Experience psychosocial functional impairment due to disability, chronic illness, or short-term health needs
- Require support with daily living activities and could benefit from service coordination
- Are living in the Frankston or Mornington Peninsula catchment.

Mentis Assist manages Home and Community Care Program for Younger People (HACC PYP) Linkages packages in the Frankston and Mornington Peninsula regions.
