Linkages Packages Program

Assistance to access a range of supports to improve or maintain your capacity to live independently

The Linkages program offers support to people aged under 65 or 50 for Aboriginal or Torres Strait Islander living with complex mental and physical health needs.

A care coordinator will work with you to assess your needs and goals before putting a care plan together that incorporates support services and community connections.

  • Referrals to various allied health services including OT, physiotherapy, psychology;
  • Referrals to local council for HACC assessments and services;
  • Assistance with accessing NDIS or My Aged Care supports;
  • Nursing;
  • Domestic assistance;
  • Maintaining/building life skills;
  • Community connections.


You may be eligible for Linkages if you require assistance due to physical and or psychosocial functional impairment related to disability, chronic illness or short-term health needs, and could benefit from assistance with daily activities.

Mentis Assist manages Home and Community Care Program for Young People (HACC PYP) Linkages packages in the Frankston and Mornington Peninsula regions.
