Mentis Assist is a registered NDIS mental health provider; we're here to support you on your NDIS journey. The NDIS may seem daunting but Mentis Assist is here to help, with 4 simple steps to get you NDIS ready.
Step 1) Find out if you are eligible for the NDIS
To be eligible for NDIS funding, you must be either an Australian citizen or permanent Visa holder under 65 years of age with a disability, including a psychosocial disability, resulting from mental illness that is likely to be permanent and has a substantial impact on your ability to manage everyday activities.
You must also live in an area where NDIS has rolled out.
Step 2) Accessing NDIS
If you meet the above eligibility criteria and you are already receiving services from a mental health community support service, you will be contacted by the NDIA via letter or telephone to confirm your eligibility and access to the Scheme.
If you are not already receiving services, you will need to contact the NDIA on 1800 800 110 to Register for the NDIS. The NDIA will send you out an Access Request Form to complete with your GP or mental health professional.
Step 3) Preparing for your planning conversation
Once you have been approved to receive a NDIS package, you will be contacted by a NDIA Planner or Local Area Coordinator who will meet with you to discuss your needs and start developing your plan.
It is important to be prepared for this meeting and to start thinking about what supports you already receive and any additional supports you may need, your goals and what outcomes you would like to achieve. This information will allow the NDIA to work out the right plan for you.
Step 4) Create a NDIS plan
Your first NDIS plan will usually last for 12 months and be the start of a lifelong relationship with the NDIS. You can request a review of your plan at any time if your needs significantly change during this time.
The Plan will establish the reasonable and necessary supports you require to meet your immediate needs and help you achieve your goals.
